The following list is a sample of the insurances we accept. It is not a comprehensive list, as plans are added and changed very often. Also, each insurance company has a variety of preferred provider organizations (PPO), health maintenance organziations (HMO), Medicare replacement (Advantage) plans, managed Medicaid plans and numerous other commercial/ governmental plans. We accept most all of these, including all major Medicaid and most Medicare Advantage plans.

Even though your insurance may not be listed below, we most likely accept it. Please contact your insurance company to verify we are in network, or give us a call and we can help.
Many times your insurance plan will require you to get a referral from your primary care doctor because we are specialists in pain management.
Also, please remember that to safeguard our patient's protected health information, all patients are required to show a valid government issued photo ID at check-in. Please also bring your insurance card to each visit.
